Shin Splints: Management and Return to Training

Shin splints, clinically known as Medial Tibial Stress Syndrome, or MTSS, are one of the most common causes of shin pain in runners, CrossFit athletes, field sport athletes and people who have recently increased their training volume.

They are usually linked to a mismatch between training load and tissue capacity.

In simple terms, the shin bone and surrounding muscles are being exposed to more impact, repetition or volume than they are currently prepared to tolerate.

This is why shin splints are rarely just a “tight muscle” problem. Stretching may help symptoms in the short term, but long-term improvement usually requires better load management, progressive strengthening and a structured return to impact.

What Are Shin Splints?

Shin splints describe pain along the inner border of the shin bone, usually caused by repeated stress through the tibia and surrounding soft tissue structures.

This often develops when training load changes too quickly. Common triggers include:

  • Increasing running distance too fast

  • Adding more jumping, skipping or plyometrics

  • Returning to training after time off

  • Changing footwear or running surface

  • Increasing CrossFit, HYROX or conditioning volume

  • Poor recovery between high-impact sessions

The issue is not always that training is “bad”. More often, the body has not had enough time to adapt to the amount of load being placed through it.

Common Symptoms of Shin Splints

MTSS usually presents as a more diffuse, aching pain along the inside of the shin.

Typical symptoms include:

  • Pain along the inner border of the shin

  • Discomfort that worsens with running, jumping or repeated impact

  • Early-stage pain that may ease once warmed up, then return afterwards

  • Tenderness over a broad area of the shin rather than one specific point

  • Symptoms that build gradually over time

In the earlier stages, symptoms may feel manageable. Many people can still train, but notice the pain returns later that day or the following morning.

This is usually a sign that the tissue is not tolerating the current training load well.

Shin Splints vs Stress Fracture

One of the most important considerations with shin pain is whether symptoms are consistent with MTSS or whether they may be progressing towards a bone stress injury.

Shin splints are usually more spread out and tender across a wider area.

A stress fracture or more serious bone stress response may present with:

  • Sharp, highly localised pain

  • Pain at rest

  • Night pain

  • Pain that worsens despite reducing training

  • Significant pain with hopping or impact testing

  • Pain that becomes more intense and specific over time

If shin pain becomes very focal, sharp or present outside of training, it should be assessed properly before continuing with impact-based exercise.

How to Manage Shin Splints

The goal with shin splints is not simply to rest until the pain disappears.

Rest can reduce symptoms, but if the underlying capacity issue is not addressed, symptoms often return when running or impact is reintroduced.

A better approach is to manage load while progressively rebuilding tolerance.

1. Reduce or Modify Impact Load

This may include temporarily reducing:

  • Running volume

  • Running frequency

  • Jumping

  • Skipping

  • Box jumps

  • High-volume burpees

  • Sprint work

  • Hard surface conditioning

This does not always mean stopping all training. The aim is to reduce the specific load that is irritating the shin while keeping the rest of the body moving.

2. Use Lower-Impact Conditioning

To maintain fitness, lower-impact options can be useful while symptoms settle.

Good alternatives may include:

  • Bike intervals

  • Ski erg

  • Rowing

  • Sled work

  • Swimming

  • Strength training with controlled loading

This allows you to maintain conditioning without repeatedly aggravating the shin through impact.

3. Build Calf and Lower Limb Capacity

A progressive strength plan should usually include work for the calf complex, ankle, foot and lower limb.

Useful exercises may include:

  • Isometric calf raises

  • Seated calf raises

  • Standing calf raises

  • Soleus-focused calf work

  • Tibialis anterior strengthening

  • Foot control drills

  • Split squats

  • Step-ups

  • Controlled plyometric progressions

The key is progression. Loading needs to be built gradually so the tissue can adapt.

4. Reintroduce Impact Gradually

Once symptoms are better controlled, impact should be reintroduced in a structured way.

This may start with:

  • Low-level pogo jumps

  • Skipping progressions

  • Short run-walk intervals

  • Hill walking

  • Shorter controlled runs

  • Gradual increases in running volume

The mistake many people make is going straight from rest back into normal training. This often leads to symptoms flaring again because the tissue has not been prepared for impact.

5. Review Training Load and Recovery

Shin splints often come down to load management.

Important questions to review include:

  • Has your running volume increased recently?

  • Have you added more conditioning or jumping?

  • Are you recovering between high-impact sessions?

  • Have you changed footwear?

  • Are you training on harder surfaces?

  • Are you combining running with CrossFit, HYROX or field sport volume?

A good rehab plan should not only reduce pain. It should also identify why the symptoms started in the first place.

When Should You Get Shin Pain Assessed?

You should seek assessment if:

  • Shin pain keeps returning when you run

  • Symptoms are becoming sharper or more localised

  • Pain is affecting your ability to train

  • You are unsure whether it is shin splints or a stress fracture

  • You have pain at rest or night pain

  • You have reduced training but symptoms are not improving

Early assessment can help prevent a manageable issue from becoming a longer-term bone stress injury.
